Since its introduction in 1891, the concept of the Chiari 1 malformation has been undergoing constant review and revision. It is often confusing for centers with limited exposure to this disorder to the cope with the advances. This confusion may be lessened by doing three things. First, we should focus on the pathophysiology of the disorder rather than its anatomy. Second, we should keep an open mind to newer concepts of treatment, such as a more conservative approach. If surgery is to be done, less aggressive dissection of the dura and arachnoid can also be considered. Finally, there should be increased awareness of the role of cervical instability in treating the patient, especially in cases that fail to respond to simple posterior fossa decompression.
