One of the leading causes of death in patients with traumatic brain injury is withdrawal of care. Historically, clinicians have not been able to reliably predict outcomes after neurological injuries. In traumatic brain injury, this has resulted in views that aggressive treatments, both surgical and medical are futile. Recent studies have demonstrated however that outcomes for a number of neurological injuries are not as poor as previously thought. This indicates a need to re-evaluate how we determine prognosis in patients with traumatic brain injury and discuss goals of care.

New treatments are emerging for the care of brain injured patients in the Intensive Care Unit. We will examine three recent trends in this field: brain tissue oxygen monitoring, autoregulation monitoring, and detection and treatment of spreading depolarizations. Using these tools, we may individualize care for these patients to achieve the most optimal outcomes.
